When you are completing the form to the Northern Territory Civil and Administrative Tribunal to apply for a guardianship order you need to include evidence that shows the person needs a guardian to help them make decisions.
It is important to have good information in the application because Guardianship has a significant impact on a person. This can be a recent medical or psychological assessment. You can ask a doctor or other health professional to complete a report with the Tribunal’s Form AG3: ‘Report by a Medical Practitioner or Other Health Practitioner’. You might also need a Form AG2: ‘Proposed Guardian Eligibility Declaration’, and Form AG4: ‘Primary Carer’s Report’.
