Book an Advance Personal Plan appointment

The Public Guardian and Trustee can help people:

  • understand the Advance Personal Plan form
  • make a new Advance Personal Plan
  • update an existing Advance Personal Plan.

To make an Advance Personal Plan you must be at least 18 years old and able to understand and make decisions about the matters covered in the plan at the time you make it.

Registering an Advance Personal Plan?

An appointment is not required to register an Advance Personal Plan that has already been completed.

Completed Advance Personal Plans may be brought into a Public Guardian and Trustee office during business hours for registration. Alternatively, you can call us on 1800 810 979 to discuss other registration options.

Appointments are available if you would like assistance to understand, prepare or update an Advance Personal Plan.

Before your appointment, it is important to think about:

  • who you want to appoint as a decision-maker
  • your views, wishes and beliefs about future
    • health care needs
    • lifestyle choices
    • living arrangements
    • financial matters, like managing property and other assets.

You should talk to your doctor, lawyer or accountant about any complex health, legal or financial matters before your appointment.

You will need to bring the following to your appointment:

  • Proof of identity (e.g. drivers licence, passport or proof of age card)
  • Previous Enduring documents or Advance Personal Plan
