The Public Guardian and Trustee can help people:
- understand the Advance Personal Plan form
- make a new Advance Personal Plan
- update an existing Advance Personal Plan.
To make an Advance Personal Plan you must be at least 18 years old and able to understand and make decisions about the matters covered in the plan at the time you make it.

Before your appointment, it is important to think about:
- who you want to appoint as a decision-maker
- your views, wishes and beliefs about future
- health care needs
- lifestyle choices
- living arrangements
- financial matters, like managing property and other assets.
You should talk to your doctor, lawyer or accountant about any complex health, legal or financial matters before your appointment.
