How to find out if an Advance Personal Plan is registered
The Public Guardian and Trustee maintains a voluntary register of Advance Personal Plans. Anyone with a proper interest may apply to check if a person has a plan registered with the Public Guardian and Trustee and obtain a copy.
Eligible individuals include:
- health care professionals
- legal professionals
- named substitute decision makers.
Please note that registration does not mean the Public Guardian and Trustee has approved the contents of the plan or confirmed that it is legally valid or able to be carried out.
